To refer yourself/someone to Headway Swindon, please follow the procedure below:
- Click here to download our referral form and send to our Client and Community Lead, Caroline: Caroline@headwayswindon.org.uk or post to: Headway Swindon, Ground Floor, Avocet House, The Dorcan Complex, Faraday Road, Dorcan, Swindon, Wiltshire, SN3 5HQ.
- Caroline will make contact and arrange an initial visit for the brain injury survivor plus their carer, family member or professional as required. The purpose of the initial visit is for us to gain more information about how your injury affects you and to look around the centre.
- If all goes well, we would then arrange a start date.
We charge a daily attendance rate. Clients need to be funded to attend and this is means tested. Clients will need to make their own way to our centre (or have transport arranged for them).
